How to Apply for Academic Qualification Documents

Melbourne Polytechnic will issue Academic Qualification Documents via My eQuals, the official tertiary credential digital platform for the Higher Education sector, students, and verifiers providing secure access to certified, official qualification documents.

Archive Investigation (Academic Qualification Documents Issued Prior to 2010) Fee
A non-refundable fee applies where an investigation is required into the Institute’s archive records. $40.00

PREVIOUSLY ISSUED (Academic Qualification Documents issued between 2010 – current)

Process to be Followed:

  1. Complete the Student Request Form as follows:
    • Check the ‘Certificates/Results’ box.
    • What is your certificate enquiry about? From the dropdown menu select ‘I would like a replacement of my qualification’.
    • Complete Course Details.
    • Any additional information can be included in the box provided.
    • Click ‘Submit’ button.
  1. Your request will be assessed by the Student Hub and onforwarded to the relevant Department for processing.
  2. Following verification of previously issued academic qualification documents, you will receive an email together with the Academic Qualification Document/s Payment Form for your completion and return.
  3. Once your payment has been processed, you will be issued with the requested documents via My eQuals or where hard copies have been requested, via Australia Post, within ten (10) business days.

Request for academic qualification documents prior to 2010

Note:

  1. In October 2014, the Institute had a legal name change from Northern Melbourne Institute of TAFE to Melbourne Polytechnic.
  2. Under the regulations, once a Training Product has been removed from Melbourne Polytechnic’s Scope of Registration, the Institute is not permitted to issue a Qualification or Statement of Attainment for a learner except as a replacement for a previously issued Qualification or Statement of Attainment.
  3. A non-refundable fee will apply where an investigation into the Institute’s archive records is required. This process can take up to 3-4 weeks.

Process to be Followed:

  1. Complete the Student Request Form as follows:
    • Check the ‘Certificates/Results’ box.
    • What is your certificate enquiry about? From the dropdown menu select ‘I want to apply for my qualification (completed before 2010)’
    • Complete Course Details.
    • Any additional information can be included in the box provided.
    • Click ‘Submit’ button.
  2. Where the student course enrolment record can be verified, you will receive an email advising the outcome and next steps.
  3. Where the student record cannot be located, an investigation into the Institute’s archive records will take place. Following the investigation, you will receive an email advising the outcome.
