Updating Your Personal Information

It is important to keep your personal details updated so that we can stay in contact and easily provide you with copies of your results and certificates.

Which details need updating How to Update

Personal details including:

  • Phone number/s or email address;
  • Address change/s;
  • Emergency contact details;
  • Preferred name change

Legal name and/or date of birth changes (Please note, before any changes can be made to legal name and/or date of birth with Melbourne Polytechnic, details must be changed with USI. For information and to complete this process please visit the USI website: https://www.usi.gov.au/.

Either

  1. Complete the online Student Request form and attach evidence as required

Evidence for legal name and/or date of birth changes: you must provide evidence of the update with USI and certified documentation such as a passport, marriage certificate, drivers licence and/or birth certificate attached to your online Student Request form. Please see information on obtaining certified copies below.

  1. Download and complete an Amendment to Personal Details Form
  2. Return the form to the Melbourne Polytechnic Student Hub at Preston or an Information Centre at any other campus alongside evidence as required.

What is a certified copy?

A 'certified copy' of an original document is a copy that has been verified as being a true copy of an original document such as a, birth certificate, marriage certificate, driver’s licence and/or passport.

This is done by a person who is authorised to certify copies of original documents. There are many professions in Victoria who are able to certify documents including: pharmacists, police officers and nurses. How to get a copy certified:

Bring both the original and a copy of the original to the certifier.

The certifier will:

  • examine the original to ensure it is not a copy or forgery
  • examine the copy to ensure it is identical to the original. A copy can be considered identical even if it is a different size or colour, so long as that does not result in the loss of any material information.
  • The certifier will then write or stamp the copy with the words: “Certified to be a true copy of the original seen by me.” They will sign and date the copy, and write or stamp their:
    • Name
    • personal or professional address, and
    • qualification as an authorised certifier.
    • If there are multiple pages to the copy, the certifier will sign or initial and number all pages.

Your document has now been certified and you can upload it as part of your application.
